Development of an arterial Spin Labeling method for brain perfusion evaluation in patients with delayed arterial transit times

The capacity of the brain to store energy is very limited so that severe reductions in cerebral perfusion may cause significant neuronal damage. This vital importance of perfusion to the integrity of brain tissue inspired the search for methods that allow its quantification. Arterial spin labeling (ASL) is a well-established method which can provide quantitative maps of cerebral blood flow non-invasively. However, there are still some methodological issues which hinder its application to specific diseases, such as, for example, those that delay the arterial transit time in specific brain regions, such as carotid stenosis and stroke. This project aims to understand how the arterial transit time affects the ASL signal and develop a methodology based on multi-phase ASL techniques that allows the evaluation of tissue perfusion even with delayed transit time. The application of the methodology in patients with carotid stenosis will enable a feasibility study as well as a clinical validation of the methodology. Thus, we intend to provide a tool which should enable more consistent evaluations regarding cerebral hemodynamics related to pathologies that delay the arrival of the blood to the brain in a completely non-invasive manner.
